Claude Handoff

Overview

Claude Handoff is a Model Context Protocol server that enables session handoff for Claude Code. It lets you list active sessions and generate handoff documentation that another agent can use to continue your work seamlessly. By exposing these capabilities as MCP tools, it integrates directly into your AI assistant workflow.

The server runs locally on your machine via the Python package claude-handoff, installed with uvx claude-handoff. It communicates over stdio and requires no authentication or external credentials. Version v0.19.0 is available now under the MIT license, with full source code at Vasilispapg/claude-handoff on GitHub.
